Instructions to Complete Your Activity Report:
Section 1. Choose report month
Section 2. Choose report year
Section 3. Select your name from the list
Section 4. Select the region you represent
Then sections 5. and 6. will appear for your input with a list of QWaLC Member Groups in your region.
Section 5. Member Group Interactions and, if applicable, Status
To report interactions you have had with specific QWaLC member groups during the reporting period select the:
1. Group Name from the list,
2. Interaction Type,
3. Activity Type or Status, and
4. Add Related Description/Comments/Outcomes.
5. Add another row for the next group and repeat the above steps (1. to 4.)
If you did not interact with any member groups simply select the option at the bottom of the Group Name list
Section 5. If applicable you can report the status of a specific QWaLC member group that is either not in your region, not operational or closed down . To do this select the:
1. Group Name from the list,
2. Choose Interaction Type: None - See Status,
3. Activity Type or Status – select the applicable status, and
4. Add any Related Comments
5. Add another row for the next group and repeat the above steps (1. to 4.)
Section 6. Non-Member Group Interactions
To report interactions with Non-Member Groups or persons during the reporting period select the:
1. Group/Person Type from the list
2. Enter the Name of Group/Person
3. Select the Interaction Type, and
4. Provide a Description of Activity/Outcomes or other details
5. Add another row for the next group and repeat the above steps (1. to 4.)
If you had no interactions of this type simply select the option at the bottom of the Group/Person Type
Section 7. Upload any photos you like together with a brief caption (optional)
Section 8. Make any additional comments or provide other information (optional)<
Submit Report: Once your report is submitted your will receive a conformation email and be able to download a pdf copy of the report.

Save and Continue Later: You will be given a link and an option to receive the link via email. Any photos uploaded will not be saved until final submission.
